Feral Ghoul

Feral ghouls represent the tragic endpoint of ghoul degeneration — humans who were exposed to massive radiation doses (or specific radiation-FEV combinations) and underwent ghoulification, only to eventually lose their cognitive faculties and devolve into aggressive, instinct-driven creatures. While many ghouls retain their intelligence and personality indefinitely (often living for centuries), those who become feral represent the condition's most feared manifestation.

Physically, a feral ghoul's appearance reflects the extreme radiation damage that created it: skin is necrotic, desiccated, and often partially sloughed away, revealing underlying muscle and bone. The nose, ears, and lips are frequently eroded or absent. Eyes are clouded or absent entirely, yet ferals demonstrate an uncanny ability to detect living creatures — possibly through scent, sound, vibration, or residual radiation signature. They move with a shambling gait at rest but can sprint with alarming speed when stimulated by the presence of prey.

The process of going feral is poorly understood. Some ghouls remain lucid for over 200 years; others degenerate within decades. Theories attribute the divergence to initial radiation dose, individual genetic factors, psychological trauma, or continued radiation exposure. What is known is that once a ghoul goes feral, the condition is irreversible, and the individual's personality is effectively dead — what remains is a radiation-zombie driven by hunger and aggression.

Feral ghouls are found wherever pre-war human populations were concentrated: urban ruins, subway systems, vaults, and military installations. They congregate in groups and are attracted to noise and light. Their primary threat is their numbers — ferals in a group can overwhelm even well-armed targets through sheer mass. Ghouls are immune to radiation and, in fact, can be healed by it, making radioactive environments particularly dangerous when ferals are present. The Glowing One variant — a feral that has absorbed so much radiation it emits a constant radiological aura — can heal and revive other ferals in its vicinity, making it a priority target.
